Financial Disclosure Report - KREF 025 (.pdf 170kb) 

This form is used by Justices and Judges of the Court of Justice, Commonwealth's attorneys, all candidates for all those offices and persons appointed to a complete an unexpired term of any of the offices mentioned above.  This form requires reporting of financial information as required by KRS 61.740.

Fundraiser Registration - KREF 019 (.pdf 60 kb)

This form must be completed when an individual has both solicited and received contributions in excess of $3,000 on behalf of a candidate for an office in a jurisdiction containing in excess of 200,000 residents.

This form is used by slates to appoint a campaign treasurer and to disclose the location of the campaign depository.

This form is used only by Slates to officially file campaign committee information.

Waiver from Filing Candidate Election Finance Statement - KREF 011 (.pdf 27kb)

This form is completed when the committee is taking responsibility for submitting the financial reports for the slate.  Both the candidate for Governor and the candidate for Lieutenant Governor should complete a separate Waiver from Filing Form.

Committee Update Form

The Committee Update form should be completed when there is a change in a committee’s designated officers or campaign depository (bank). Kentucky law requires that a committee designate a chairperson and treasurer (who must be separate persons), and an official contact person. NOTE: Executive agency lobbyists (as defined in KRS 11A.201) and legislative agents (as defined in KRS 6.611) are prohibited from serving as the official contact person of a permanent committee. In addition, if there is a vacancy in a committee’s treasurer, the chairperson shall be accountable as the committee’s treasurer, if the vacancy is not filled within three (3) days following the treasurer’s notice of resignation. See KRS 121.170.

Unauthorized Campaign Committee Forms

Political Committee Registration – KREF 010

This form should be completed to register as an Unauthorized Campaign Committee.

Election Finance Statement - KREF 006

Unauthorized Campaign Committees use this set of forms for reporting financial transactions of the committee to the Registry.

Instructions for Completing Election Finance Statement KREF 006

These instructions show an Unauthorized Campaign Committee how to complete the schedules in the Election Finance Statement. Take note that contribution limits do not apply to Unauthorized Campaign Committees.

Report of an Independent Expenditure - KREF 013

Independent Expenditures by an Unauthorized Campaign Committee are not subject to any limits, but are subject to reporting requirements. Unauthorized Campaign Committees who make Independent Expenditures exceeding five hundred dollars ($500) in the aggregate in any one election are required to report the independent expenditures directly to the Registry.
